A nexus letter is a written opinion based on the records - the service treatment records, VA treatment notes, private records, and lay statements that document the condition and the in-service event. What gives the opinion its weight is the author's qualifications, the completeness of the record reviewed, and the quality of the reasoning, not the author's zip code. Why location rarely matters for a written opinion
Raters weigh medical opinions on their rationale, not their return address. A physician who has reviewed the complete claims file and explained the medical mechanism connecting condition to service produces probative evidence from anywhere in the country. This is also how the VA itself works: C&P examinations are routinely performed by contractors states away from the veteran, and many VA opinions are records-only reviews with no examination at all.

Where veterans start
Every case is different, and there is no obligation at any step. When veterans want to understand what their records actually support, these are the two most common starting points:

- Nexus letter
When the records support one, a licensed provider writes the medical opinion that addresses the link between your condition and your service or an already service-connected disability.
The one real caveat: mental health evaluations
Psychological evaluations are different, because they involve a live clinical evaluation rather than a records-only review. Psychologists are licensed state by state, and a telehealth evaluation must be conducted by a psychologist authorized to practice where you are located. When an in-person examination still helps
